I understand that:

The Education and Training Reform Act 2006 requires National Training Pty Ltd to collect and may disclose my personal information for a number of purposes including the allocation of a USI and updating my personal information if required. National Training complies with the Privacy Act 1988. Your data will only be disclosed to state and federal regulatory bodies for mandatory data reporting and when required for audit purpose. Under the Data Provision Requirements 2012, National Training is required to collect personal information about you and to disclose that personal information to the National Centre for Vocational Education Research Ltd (NCVER). Your personal information (including the personal information contained on this enrolment form), may be used or disclosed by National Training for statistical, administrative, regulatory and research purposes. National Training may disclose your personal information for these purposes to:

• Commonwealth and State or Territory government departments and authorised agencies; and
• NCVER

Personal information that has been disclosed to NCVER may be used or disclosed by NCVER for the following purposes:

• populate authenticated VET transcripts;
• facilitate statistics and research relating to education, including surveys and data linkage;
• pre-populate RTO student enrolment forms;
• understand how the VET market operates, for policy, workforce planning and consumer information; and
• administer VET, including program administration, regulation, monitoring and evaluation.

You may receive a student survey which may be administered by a government department or NCVER employee, agent or third party contractor or other authorised agencies. Please note you may opt out of the survey at the time of being contacted.

NCVER will collect, hold, use and disclose your personal information in accordance with the Privacy Act 1988 (Cth), the National VET Data Policy and all NCVER policies and protocols (including those published on NCVER’s website at www.ncver.edu.au).

I understand that:

When we apply for a USI on your behalf the Registrar will verify your identity. The Registrar will do so through the Document Verification Service (DVS) managed by the Attorney‐General's Department which is built into the USI online application process if you have documents such as a Medicare card, Australian birth certificate, Australian driver’s licence, Australian passport, citizenship document, certificate of registration by descent, ImmiCard or Australian entry visa. If you do not have a document suitable for the DVS and we are authorised to do so by the Registrar we may be able to verify your identity by other means. If you do not have any of the identity documents mentioned above, and we are not authorised by the Registrar to verify your identity by other means, we cannot apply for a USI on your behalf and you should contact the Student Identifiers Registrar. In accordance with section 11 of the Student Identifiers Act 2014 Cth (SI Act), we will securely destroy personal information which we collect from you solely for the purpose of applying for a USI on your behalf as soon as practicable after the USI application has been made or the information is no longer needed for that purpose, unless we are required by or under any law to retain it. The personal information about you that we provide to the Registrar, including your identity information, is protected by the Privacy Act 1988 Cth (Privacy Act). The collection, use and disclosure of your USI are protected by the SI Act. If you ask National Training to make an application for a student identifier on your behalf, National Training will have to declare that National Training complied with certain terms and conditions to be able to access the online student identifier portal and submit this application, including a declaration that National Training to has given you the following privacy notice: You are advised and agree that you understand and consent that the personal information you provide to us in connection with your application for a USI: Is collected by the Registrar for the purposes of: Applying for, verifying and giving a USI; Resolving problems with a USI; and Creating authenticated vocational education and training (VET) transcripts;

May be disclosed to: Commonwealth and State/Territory government departments and agencies and statutory bodies performing functions relating to VET for: The purposes of administering and auditing Vocational Education and Training (VET), VET providers and VET programs; Education related policy and research purposes; VET Regulators to enable them to perform their VET regulatory functions; VET Admission Bodies for the purposes of administering VET and VET programs; Current and former Registered Training Organisations to enable them to deliver VET courses to the individual, meet their reporting obligations under the VET standards and government contracts and assist in determining eligibility for training subsidies; Schools for the purposes of delivering VET courses to the individual and reporting on these courses; The National Centre for Vocational Education Research for the purpose of creating authenticated VET transcripts, resolving problems with USIs and for the collection, preparation and auditing of national VET statistics; Researchers for education and training related research purposes; Any other person or agency that may be authorised or required by law to access the information; Any entity contractually engaged by the Student Identifiers Registrar to assist in the performance of his or her functions in the administration of the USI system; and will not otherwise be disclosed without your consent unless authorised or required by or under law;

FNS50217 Diploma of Accounting entry requirements

To start the Diploma of Accounting, you’ll need to have either a Certificate IV in accounting or a bookkeeping OR the FNSSS00014 Accounting Principles Skill Set which includes the following units:

• FNSACC311 Process financial transactions and extract interim reports
• FNSACC312 Administer subsidiary accounts and ledgers
• FNSACC416 Set up and operate a computerised accounting system
• FNSTPB402 Establish and maintain payroll systems
• FNSTPB401 Complete business activity and instalment activity statements
• FNSACC408 Work effectively in the accounting and bookkeeping industry

Terms and Conditions

• I confirm that the information provided to the RTO in this application is to the best of my knowledge true, correct and complete at the time of my enrolment/application.
• I confirm that I will abide by National Training’s Policy & Procedures and I have read and understood the Student Handbook .
• I understand that providing any false information and/or failing to disclose any information relevant to my application for enrolment and/or failure to complete an application /enrolment form may result in the withdrawal of any offer, and/or cancellation of enrolment at the discretion of National Training.
• I understand that it is my responsibility to provide all relevant and required documentation.
• I understand and accept the course fees and refund policy as detailed on National Training website.
• I understand that the course fees are non-transferable and should you wish to cancel your enrolment within five (5) days of the payment date, National Training will refund course fees paid.
• I understand if I enter into a payment plan contract to pay for my course and choose to cancel within the five-day cooling-off period I will be required to pay the setup cost, which is $250 for a weekly plan and $150 for a monthly plan.
• I understand that in the additional copies of Certificate and or Statement of Attainment will cost $50.00 per certificate.
• I authorise National Training (NT), or its agent, in the event of my illness or any accident that befalls me while I am at a NT premises or during any organised activity to seek ambulance, medical, or surgical treatment at my cost.
• In the event I do not have a Unique Student Identifier (USI), I authorise National Training to apply for a USI on my behalf.
• I authorise National Training Pty Ltd to obtain any information regarding my enrolment and participation in any training and assessment program provided by any educational institution. This information may include: details of qualifications obtained, Statements of Attainment, Statements of Results and dates on which these were achieved and awarded.
• I have read and understood all the information contained in the Student Handbook and submission of assessment terms and conditions
• I have read and understood the complaints process, refund policy and withdrawal policy as contained in the Student Handbook and on National Training's website.
• I am aware of the delivery method for the qualification I am applying for and declare I have access to a computer, internet and email to complete my course online.
• I am aware there may be a requirement for me to conduct roleplays and declare I have access to a video recording device, uploading abilities and individuals who will assist in roleplay activities as required.
• I authorise National Training to use my image, content and or survey responses in marketing materials including, but not limited to National Training's website.
• I agree to participating in, and providing feedback to the RTO by way of survey on completion of enrolled qualification/s
• I understand that the course is self-paced, and I agree to complete at least one (1) unit every ninety (90) days, and that National may cancel my enrolment if I do not comply with this requirement.
• I understand that if no work has been completed for the previous twelve (12) months, my enrolment will be cancelled under cessation of studies. I understand I can request a six (6) month extension, at a cost of $400. Course extension will only be considered in cases that are not effected by transition. Extension fees may be waived under unique and compelling circumstances, at the discretion of the CEO.
• All qualifications are subject to transition into a new qualification code at any time without notice. Should the qualification you are enrolled in, be updated by the Australian Government, you will be required to complete your qualification within twelve (12) months of being notified of transition.
• I understand I will be required to participate in the facilitation of survey completions and or roleplays, requiring the interaction with others, such as: family friends or colleagues throughout the duration of my course.
